A Springfield woman died this weekend from a four car crash in Mattoon. 

The accident occurred at the intersection of Route 16 and Lerna Road around 8:30 Saturday night. 

Three cars were coming to a stop at the intersection when 25-year-old Michael Fogarty of Chicago rear-ended the car in front of him causing a chain reaction that killed 40-year-old Amy Thomas of Springfield. 

Fogarty received several citations including driving too fast for conditions, failure to reduce speed, no seat belt, and a DUI.
